If expired more than 5 years you need to apply for a New Passport

To report a lost or stolen passport, you need to complete and submit Form DS-64 (Statement Regarding a Lost or Stolen Passport)

The non-applying parent must complete this form, get it notarized, and provide a photocopy of their ID front and back.

If your record was issued before 1994, you will need to pay a $150 file search fee. We charge this fee to manually search for your paper record.

You may be asked later to pay the $150 fee for a manual search if the Dept of State cannot find your record after searching for it in an electronic database.

Please use this option if we sent your passport and you have not received it within two weeks of the mailing date.
