1. Passport Application: Print and complete your Form DS-11 Application for U.S. Passport
2. Passport Photos: Can be taken at a CVS, Walgreens, Staples, or a Post Office.
3. Photocopy your Proof of Identity: Driver's License or State ID.
4. For Child Passports, under 16 years of age, provide a photocopy of both parents' ID front and back.
5. U.S. Citizenship documents: (Birth Certificate, Naturalization Certificate, and/or Expired Passport if one was issued to you previously)
6. Passport Fees: (See Below under expediting Fees)

Statement of Consent: Only for a Child Passport:

• If one parent or guardian cannot go with the child to apply in person for the child's passport
• The non-applying parent must complete this form, have it notarized, and provide a photocopy of their Driver's license or State ID front and back.
